Application Process

HOLIDAY HOURS:: The NTCB Office will be closed 12 Noon December 23 – January 4 to celebrate the Holiday Season. If we received your application/recertification form prior to December 21, your certificate(s) will be mailed out by December 23. Please allow 3 weeks for delivery. All forms received during the holidays will be processed after January 4, 2010. Thank you.

In order to receive certification, please allow 4-6 weeks for processing.

1. Determine the type of certification you're requesting.

2. Determine the correct application and fee (listed below).

3. To mail or fax your application:

• Print and complete correct application
• Attach appropriate documention to verify your credentials
• Mail your check or money order along with application to:
  NTCB, PO Box 103, Swannanoa, NC 28778
• Or fax application and all documents to our secure fax line, 888-484-3113. Payment can be either sent by check or money order or you can pay online.

Please Note:To send a fax to NTCB, please dial 888-484-3113. Since this is a dual line --- both phone and fax --- as soon as you hear our phone ringing please press START on your fax machine. This will ensure that our fax machine picks up promptly instead of putting you through to the phone line. If you have any problems please call 800-710-1539 ext 1 and we'll be happy to help you.

Also: If you prefer to pay by credit card through the mail, don't forget to include your credit card number and expiration date on the application.

4. To pay online:

• Complete correct application
• Either
  • email application and attach scanned required documentation to
    ntcb.info@ gmail.com (no spaces),
  • fax application and required documentation to our secure fax line, 888-484-3113,
  • mail application and required documentation to
    NTCB, PO Box 103, Swannanoa, NC 28778
• Send credit card payment through IPX Services Secure Credit Card Gateway
• OR send payment thru PAY PAL:

Purpose of Payment

Applications & Fees

Please enclose $25.00 for all past due recertifications. You must download Adobe Acrobat Reader to read or print the following applications.

Note 1: If you wish to order a new or replace a lost or damaged certificate issued through the NTCB, please send $25 along with your request.

Note 2: You must contact the NTCB within 3 months of applying if your certificate did not arrive. Those inquiring after 3 months are requested to pay a $25.00 service fee. It is the responsibility of the member to keep the NTCB informed of their current mailing address and other contact information.

Note 3: If you are reinstating your certification after expiration, you must enclose an additional $25 reinstatement fee in addition to the certification fee. If you are 1 year or more past due, please contact the NTCB Office by email or phone to determine your status and fees.
